Treatment

Bobath therapy is a treatment method from physiotherapy, occupational therapy and speech therapy. The Bobath concept is based on the assumption that intact brain regions can perform and relearn the tasks of destroyed or diseased regions. Movement sequences are retrained in practice through constant repetition.

Children and adults with a disorder of the central nervous system are treated according to the Bobath principle. In adults, this therapy is usually used after a stroke with signs of paralysis to improve movement sequences and transitions as well as balance reactions. The targeted treatment technique increases body awareness and makes it easier for patients to cope with everyday life. Independence and a significantly improved quality of life in daily activities are the result.

Children often receive Bobath therapy in Munich for developmental delays. This includes age-appropriate motivation through movement experiences and stimuli that optimize motor learning. In this way, children with coordination, movement and perception disorders are helped to achieve an ideal development process. Even adults and children with multiple and severe disabilities can make significant progress with Bobath therapy. In these cases, the main aim is to make movement transitions and movements easier and to maintain vital functions. Special attention is paid to individual restrictions.

Treatment goals

  • Improvement and promotion of fine and gross motor skills
  • Regulation of muscle tone
  • Avoidance of contractures
  • Increased coordination skills
  • Promotion of sensorimotor skills and motor skills

The application takes place under any health aspect, regardless of how severe the nerve damage actually is. Anything that helps the patient to regain their normal behavioral habits can become part of the treatment concept. In practice, three treatment techniques are used in Bobath therapy. Stimulation, inhibition and facilitation merge depending on the focus of treatment – they influence different senses and perceptions.

Disease patterns

Treatment according to the basic teachings of Bobath in our physiotherapy practice in Munich is the problem-solving approach for patients with a lack of movement and postural control as well as functional disorders caused by a lesion in the central nervous system. The following clinical pictures relate to this treatment method:

  • ALS Parkinson’s disease
  • Brain tumors (meningiomas, astrocytomas, oligodendrogliomas)
  • Multiple sclerosis (MS)
  • Stroke, cerebral infarction, stroke
  • Paraplegic syndromes
  • Craniocerebral trauma
  • Developmental disorders and delays
  • Genetic changes in the nervous system
  • hypoxic brain damage

In physiotherapy, the Bobath concept is considered to be one of the most successful patterns in the care and therapy of patients with cerebral movement restrictions. Consistent use prevents pain, restores the patient’s former independence and improves the interaction between the paralyzed and healthy side.

In contrast to other forms of therapy, Bobath in Munich does not have any ready-made lists of defined exercises; instead, the method is subject to constant further development with the affected patients themselves. However, only scientifically proven procedures may be added in therapeutic use. The everyday relevance for the person concerned plays a central role. By incorporating age and neurological foundations, Bobath therapy benefits from the changeability and holistic approach to the person seeking help. Depending on the type of reaction to passive movements by the therapist, an efficiency assessment of the method is provided.

The Bobath concept is used specifically for people who suffer from lesions of the first motor neuron. All exercises are based on the concept-specific assessment of findings and the process of resolving movement, tone and function disorders. In this context, the use in spastic and flaccid hemiparesis has proved particularly successful.

Billing (by health insurance company)

The Bobath concept is carried out as part of physiotherapy and is tailored to the needs of the patient. This special type of treatment is used for children and adults and is primarily aimed at people who suffer from impaired balance, perception or motor skills.
This form of physiotherapy is internationally recognized and is reimbursed by both statutory and private health insurance companies in Germany.

The treatment can be covered by a doctor’s prescription, whereby the physiotherapist can decide for himself which procedures to include in the measures.

As Bobath’s learning program does not follow a specific pattern, doctors are extremely flexible in their prescriptions. From correct positioning, mobilization and handling to self-help training, those affected benefit from all the exercises that make sense from a physiotherapeutic point of view. The contents of the Bobath concept are so variable that every patient can expect full cost coverage according to this prescription.

  1. Please send us a corresponding prescription from your doctor.
  2. We will take care of the health insurance approval for you.
  3. After treatment you will receive an invoice from us.
  4. Reimbursement will be made either directly (statutory health insurance) or after submission of the original referral including paid invoice and approved referral (private health insurance).

However, in order to receive good outpatient care, longer and more intensive physiotherapy according to the Bobath concept is often required. In this case, health insurance companies only pay for basic care, including the necessary remedies. However, the path back to an independent life can be much more difficult. You should therefore be prepared for additional costs or check whether your private accident or health insurance will cover part of the costs.

Background / History

The Bobath couple began developing their own form of therapy back in the 1940s. Initially, they used the treatment measures for spasticity, but in the course of further development it became apparent that the method can also optimally promote and improve other damage to the central nervous system.

Berta Bobath, a physiotherapist who emigrated from Berlin to England, gave her name to the method and recognized that spasticity can be influenced by different positions, positioning and movements. Based on this knowledge, her husband, neurologist and paediatrician, Dr. Karl Bobath, developed the neurophysiological foundations for the successful Bobath concept.

Surprisingly, doctors had previously assumed that spasticity was untreatable and that constant damage to the brain and nerves sealed the patient’s fate. Berta Bobath proved the opposite with her concept. However, in order to investigate the phenomenon in more detail, she tested various treatment options in the 1940s and founded a private clinic in London in 1951 as an alternative treatment for movement disorders.

Particularly at the beginning, only affected parts of the body were treated and the application was rather mechanical and passive. However, the focus increasingly shifted to the independence of patients and so the original theory developed into a holistic treatment concept.

Today, the focus of treatment is on preventing secondary changes such as joint stiffness. The entire perceptual function of the human body with its motor learning abilities at all levels also plays a key role in relearning movement skills. The advantages for patients result from the intensive cooperation of the person concerned and a reduced need for care, which can prevent dependency and placement in care facilities.
